Objective 1: To enhance my diagnostic abilities, I will conduct clinical assessments on at least five patients each week for the next ten weeks using the diagnostic criteria found in the fifth edition of the DSM-V.
Planned Activities: During my regular hourly check-ins, I will thoroughly review each patient’s health records to minimize preventable interruptions in the provision of medication (Aggarwal et al., 2022).

Objective 2: After ten weeks, I will have improved my pharmacological knowledge by understanding how to give pharmaceuticals based on the patient’s age, as well as their classifications, uses, adverse reactions, and interactions, by evidence-based clinical practice guidelines.
Planned Activities: Analyze drug classification, interactions, and administration techniques. When creating a treatment plan, consider the side effects and circumstances in which the medication should not be administered (Kaas, 2019).

The Nursing Theory and Counselling Theory
In nursing, theories are essential because they provide organized practice frameworks and function as guiding concepts for practice tasks. I have chosen Person-Centered Therapy as the counseling theory and Callista Roy’s Roy Adaptation Model as the nursing philosophy for my clinical work. The Roy Adaptation Model emphasizes the interconnection of a person’s psychological, social, and biological systems while concentrating on that person’s ability to adapt to internal and external influences. This concept applies primarily to clinical settings when patients encounter intricate health issues necessitating thorough evaluation and solutions. This paradigm promotes adaptation, thereby contributing to maintaining the state of well-being and health while encouraging broad patient care. Studies have shown that using the Roy Adaptation Model can positively impact patient outcomes, as it promotes the development of effective coping mechanisms and enhances patients’ overall care (Morrow & Roy, 2022).
Person-centered therapy (PCT) emphasizes the significance of therapeutic connections and an individual’s self-directed development. PCT fosters a receptive and encouraging atmosphere, allowing participants to articulate their views, experiences, and emotions freely. Implementing this method is crucial for cultivating a therapeutic relationship, which strongly indicates favorable treatment results. The approach promotes authenticity, unwavering acceptance, and understanding from the therapist, fostering clients’ personal growth and resilience (Tudor, 2022). Integrating the Roy Adaptation Model with Person-Centered Therapy is crucial for delivering comprehensive and patient-focused therapy. This combination facilitates addressing patients’ mental and physical requirements, boosting their total health and well-being.
Part 3: Projected Timeline/Schedule
I intend to complete the 144 or 160 Practicum hours (as applicable) according to the following timeline/schedule. I also understand that I must see at least 80 patients during my practicum experience. I may not complete my practicum hours sooner than eight weeks. I may not be in the practicum setting longer than 8 hours per day unless pre-approved by my faculty.
